Overview

As an Integrated Lead, you will provide operational leadership across ICAT services, coordinate care pathways, and work collaboratively with multidisciplinary teams to deliver integrated, outcome‑focused support.

Key Responsibilities
  • Provide operational leadership across ICAT services.
  • Ensure effective coordination of care pathways, supporting timely interventions and safe transitions.
  • Work collaboratively with multidisciplinary teams to deliver integrated, outcome‑focused support.
  • Drive service improvements aligned with Adult Social Care priorities and statutory requirements.
  • Lead and manage staff within the integrated teams, fostering a culture of collaboration and continuous improvement.
  • Monitor performance and quality standards, ensuring compliance with policies and best practice.
  • Support strategic objectives for short‑term interventions, reducing hospital admissions and promoting independence.
  • Engage with stakeholders, including health partners, to strengthen integrated working.
Qualifications & Requirements
  • You must be registered as a Social Worker with Social Work England, or have applied to register with the Social Care Council.
  • Experience in integrated care and short‑term support is highly desirable.
  • Demonstrated leadership, service improvement, and partnership working.
  • Ability to provide a company car for duty or be insured for business use (exceptions may be considered for disability).
