Salary: £36,363 - £40,777 per annum. Position:
Permanent, Part Time Social Worker in the Short Term Support Service.

This is an incredibly exciting time to join our service. We have launched Living Better Lives in Lancashire, our new strengths‑based vision for Adult Social Care. We are looking for a Social Worker to support us to deliver our vision of supporting people to achieve what matters to them, living independently in a place they call home.

This post is a weekend working position, Friday to Monday (29.6 hours).

Joining the Short Term Support Service as a Social Worker, you will support safe, timely and person‑centred discharges from hospital, working closely with health colleagues, families and wider partners. You will help people move from hospital to the most appropriate setting, promoting recovery, independence and a positive discharge experience.

Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.
