About the RoleWe are looking for a family support worker to join the Children and Family Wellbeing Service. The role involves working alongside a small, dedicated team across Lancashire, providing long‑term support for children and young people with special educational needs and disabilities (SEND) and their families.
Key responsibilities include:
- Providing specialist, personalised support to children and young people with SEND and their families.
- Working in partnership with education, health, and care providers to facilitate the child's learning and development.
- Supporting the implementation and review of inclusion plans that are co‑produced with families.
- Coordinating service delivery alongside other key partner agencies through a Team Around the Family approach.
- Ensuring that children and young people are supported to achieve, thrive, and reach their full potential.
