Base pay: $26.33/hr - $37.18/hr.

• Participation in the development of program plans for individuals; monitoring, evaluating, and revising these plans as necessary.
• Identify and coordinate services for individuals and their families.
• Complete documentation, forms, and reports in accordance with regulations and NLACRC policies and procedures.
• Advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals, family members, service providers, advocate, etc.
• Identify barriers to service delivery and implement culturally sensitive strategies embodying empathy, trust, and respect.
• Assist with rotating officer-of-the-day duties and provide case coverage and emergency on-call services as required.

Bachelor’s degree in psychology, social work, sociology, or related human services field and at least two years of related experience. A Master’s degree may be substituted for experience.

• Understanding of family systems and person-centered planning.
• Knowledge of intellectual and developmental disabilities.
• Excellent organizational skills and ability to work independently and as part of an interdisciplinary team.
• Excellent verbal and written communication skills.
• Proficiency with MS Word and MS Outlook.

Must be comfortable working independently, meeting clients in homes, community, or office, frequent telephone, virtual, and out-of-office meetings required, and hold a valid California Driver’s License with reliable transportation.
