Base pay range: $/hr - $/hr
Schedule: Monday-Friday with flexibility to adjust based on patient/partner needs

RoleAs a Nurse Practitioner at Greater Good Health, you will provide exceptional care to senior Medicare patients. Your responsibilities include:
- Support highly complex patients, assisting them in navigating a challenging healthcare system
- Provide patient education, create dedicated care plans, and offer clinical guidance to an integrated care team
- Work closely with the Integrated Care team to provide superior care for our dialysis patient population
- Act autonomously and collaboratively with all healthcare team members to coordinate high‑quality, cost‑effective care
- Coordinate care services among the patient, family, physicians, and other providers
- Administer various tests and screenings
- Perform comprehensive health and risk assessments with accurate clinical coding and documentation
- Maintain strict confidentiality in line with HIPAA regulations
This full‑time position includes in‑clinic visits at dialysis centers, nephrology practices, and in‑home care within your designated service area, with occasional telehealth appointments. Flexibility across regions may be required to accommodate scheduling changes.
Experience and Qualifications- Current California Nurse Practitioner license
- Board Certified NP in good standing
- NP certification from ANCC or AANP
- 2+ years of clinical NP experience in primary care, family medicine, internal medicine, gerontology, or palliative care (home care experience a plus)
- Knowledge of chronic care management and medication options
- Post‑hire DEA licensure / Prescriptive Authority
- Active CPR/BLS Certification
- Experience with HCCs
- Valid California driver’s license
