Patient Placement Coordinator - Nursing Administration - Full Time/Nights
Antelope Valley Hospital’s (AVH) Patient Placement Coordinator (PPC) is responsible for expediting patient admissions from all avenues (Emergency Department [ED], Direct Admits, PACU, Cath Lab, etc.) as well as inpatient transfers, in a timely manner, focusing on Patient Throughput targets with clinical oversight in a confidential manner. The PPC screens for level of care appropriateness and any special requirements around bed requests (e.g., Isolation, custody, dialysis, patient safety, etc.).
This role serves as the point person for appropriate patient placement, working closely with Bed Control and the House Supervisor, and reviews patient history and medical needs to direct appropriate bed placement based on current and historical data.
- Demonstrate clinical knowledge and expertise to make sound decisions for specific patient populations, reviewing electronic medical records and clinical indicators to ensure appropriate placement.
- Focus on isolation, dialysis, involuntary holding and patient safety needs.
- Communicate effectively between House Supervisor, Bed Control and Charge Nurses about patient population needs.
- Adapt workflow to meet patient‑specific needs in a timely manner.
- Direct Bed Control staff to facilitate timely throughput of admissions and transfers, ensuring correct bed placement the first time.
- Collaborate with Bed Control and House Supervisor regarding level of care, special needs and unit capacity, ensuring policies and protocols are followed.
- Manage incident command during crisis situations, coordinating with Nursing Administration.
- Maintain communication and review schedules for scheduled surgeries, Interventional Radiology, Cardiac Cath Lab and same‑day surgery patients to ensure post‑op placement.
- Assign specialized ICU/PCU beds a day before admission to secure planned placement.
- Screen all admission and transfer requests from other facilities, coordinating acceptance with House Supervisor, ED and ED physicians.
- Act as liaison with other departments, tracking delays and maintaining communication with nursing and other disciplines. Round units as needed.
- Coordinate Physician orders for all admissions and transfers, including special needs such as isolation or psychiatric consults.
- Solve problems collaboratively, escalating issues to Manager or Director as needed.
- Track hospital census and bed availability, including turnover and patient flow.
- Communicate with Charge Nurses about admissions/transfers and elevate concerns to House Supervisor or Physician when needed.
- Elevate patient holding in ED beyond three hours, documenting delays.
- Prioritize admissions, interpret bed availability, set placement priorities, and update status as unit situations change.
- Transfer patients based on Physician orders and clinical review.
- Notify Bed Control and relevant units when beds become clean and available.
- Coordinate critical care unit priorities, clearing beds a day prior when needed.
- Monitor patient movement throughout the hospital, using clinical judgment.
- Track discharges to obtain census data and coordinate quick bed cleaning with EVS Lead.
- Review Physician suspension lists before admitting patients, escalating as needed.
- Display flexibility and adaptability to new or unusual situations.
- Cover duties in Nursing Administration, maintaining confidentiality of protected information.
- Pre‑register or register admissions, verifying accurate demographic data.
- Initiate required documentation for AVH BED polling (Reddi Net) to meet county timelines, maintaining >90% timeliness.
- Review Bed Huddle information from Charge Nurses at 09:00 and 21:00, sending reports to designated groups.
- Interpret and predict placement needs for Code calls (e.g., trauma, stroke, ELVO) and clear beds for higher level of care.
- Contribute ideas and solutions to continuously improve processes and meet throughput metrics.
