Mayflower Gardens, Lancaster, CA Pay Range: $23.00 - $24.00 per hour

Are you passionate about helping older adults access the services and resources they need to thrive? RHF is seeking a Service Coordinator to support residents at Mayflower Gardens, a 502-unit senior residential community in Lancaster, CA.

In this role, you will help identify resident needs, connect residents with community resources, coordinate supportive services, and develop programs that promote health, wellness, independence, and quality of life. The ideal candidate is compassionate, organized, resourceful, and comfortable building strong relationships with residents, service providers, and community partners.

What You'll Do

Resident Services & Advocacy

  • Assess resident needs and connect residents with appropriate social, health, wellness, and community resources.
  • Provide referrals, support, and advocacy while promoting resident independence and self-advocacy.
  • Provide information and assistance during crisis situations and conflict resolution.

Service Coordination & Programs

  • Build relationships with community agencies and service providers to expand resources available to residents.
  • Coordinate and monitor resident referrals, follow up on services, and maintain a current directory of community resources.
  • Develop and coordinate educational, health, and wellness programs based on resident needs.
  • Serve as a liaison between residents, management, staff, and community service providers.

Documentation & Reporting

  • Maintain accurate and confidential records of resident interactions, referrals, and services.
  • Prepare daily and monthly reports related to resident services and activities.

Additional Duties

  • Perform other duties as assigned in support of RHF's mission and the needs of the community.
Qualifications
  • Bachelor's degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field is preferred.
  • Ability to speak, read, and write Chinese is preferred.
  • Relevant work experience supporting older adults or individuals with disabilities will be considered.
  • Knowledge of supportive services and community resources for older adults.
  • Strong advocacy, problem-solving, communication, and interpersonal skills.
  • Ability to maintain confidentiality and demonstrate sound ethical judgment.
  • Ability to work effectively with residents, staff, service providers, and community partners.
  • Strong organizational and time-management skills with the ability to work independently.
  • Proficiency with Microsoft Office, including Word, Excel, PowerPoint, and Outlook.
Physical Requirements
  • Ability to move throughout the community and interact with residents and service providers.
  • Ability to communicate effectively in person and by phone.
