Case Manager – Healthcare Navigator

Join Lutheran Social Services of Central Ohio, a leading non‑profit supporting families and veterans across 27 Ohio counties.

As a Healthcare Navigator for Faith Mission of Fairfield County, you will help clients secure stable housing, access health care benefits, and build pathways to self‑sufficiency.

• Develop client action plans focused on housing first and long‑term safety.
• Coordinate referrals to community services and VA health care benefits.
• Maintain accurate case records, weekly CQI reports, and documentation of interactions.
• Participate in weekly case review meetings to advance client outcomes.
• Provide crisis intervention and de‑escalation leadership to clients and colleagues.

• Four‑year degree in Social Work or related field (experience may substitute).
• Licensed Social Worker preferred; current Ohio driver’s license required.
• Minimum two years in a social service setting, experience with homelessness, mental health or related services strongly preferred.
