The Per Diem Registered Nurse (RN) provides skilled nursing services to Medicare home health patients in their homes. The RN performs patient assessments, implements physician-ordered care plans, provides patient and caregiver education, coordinates care with healthcare providers, and maintains timely clinical documentation.

• Skilled Nursing Care
• Perform comprehensive nursing assessments and reassessments
• Regularly evaluate patient nursing needs and response to treatment
• Implement and update the established plan of care
• Provide skilled nursing services in accordance with physician orders
• Deliver preventive, restorative, and therapeutic nursing interventions
• Identify changes in patient condition and take appropriate clinical action
• Educate patients and caregivers on disease management, medications, and self-care techniques
• Participate in discharge planning and transition of care activities
• Care Coordination
• Communicate patient status changes to physicians and obtain orders as needed
• Coordinate care with therapists, aides, social workers, and other healthcare professionals
• Collaborate with community resources to support patient goals and outcomes
• Advocate for patient needs and quality care
• Documentation & Compliance
• Complete accurate and timely clinical documentation
• Maintain compliance with Medicare regulations and agency policies
• Follow HIPAA and patient confidentiality requirements
• Participate in agency quality improvement initiatives

• Graduate of an accredited school of nursing
• Current Ohio Registered Nurse (RN) license
• Current CPR certification
• Valid driver's license
• Reliable transportation with current automobile insurance
• Strong assessment, communication, organizational, and problem-solving skills
• Ability to work independently in a field-based environment

• Home Health experience
• OASIS experience
• Case Management experience

• Frequent walking, standing, bending, and reaching
• Ability to lift, transfer, or assist patients as necessary
• Ability to travel throughout the Lancaster and surrounding service area

Field-based position serving patients in:

• Lancaster, OH
• Fairfield County
• Surrounding communities

Per Diem / PRN
