Internal Medicine - Geriatrics Physician

Position Details:

• Location:
  
  Lancaster, PA

• Setting:
  Skilled Nursing Facilities (SNF), both short-term and long-term care

• Schedule:
  
  Monday through Friday, full-time

• Flexibility:
  Autonomy to set your daily rounding schedule

Key Responsibilities:

• Provide primary care in post-acute and long-term care settings

• Manage chronic and acute conditions, prescribe and monitor medications

• Order and interpret lab and diagnostic results

• Collaborate with interdisciplinary healthcare teams

• Educate patients and caregivers on condition management and healthy lifestyle changes

• Build strong relationships with patients, families, and facility staff

• Participate in care coordination and promote smooth care transitions

Qualifications:

• MD or DO;
  Board Certified or Eligible in Internal Medicine, Family Medicine, or related primary care field

• Active, unrestricted Pennsylvania medical license

• DEA registration with prescriptive authority in Pennsylvania

• Minimum of 1 year primary care experience required

• Post-acute care experience preferred but not required
