The children's community nursing (CCN) service has an exciting opportunity to recruit to a permanent band 5 staff nurse. The CCN service is part of the integrated children's nursing and therapies team (ICNT), providing holistic nursing care to children / young people in their own homes or community settings. The role focuses on delivering clinical interventions, preventing hospital admissions, supporting early discharges, and teaching families to manage their child's health care needs independently.
We are looking for an enthusiastic and highly motivated band 5 staff nurse with excellent communication skills to assess, plan, implement and evaluate individualised nursing care plans for children with acute, chronic or life-limiting conditions.
You will need to work autonomously and also collaboratively with other professionals and actively contribute to multidisciplinary team (MDT) meetings where necessary, for example, discharge planning, transition, and safeguarding.
You will be joining an enthusiastic and motivated ICNT team who are approachable and supportive.
Main duties of the job- Assess, plan, implement and evaluate individualised, evidence-based nursing care plans for children with acute, chronic and life‑limiting conditions in their own homes or community settings.
- Develop and maintain competencies in agreed clinical skills in designated area.
- Provide family support and education, including feedback, guidance and training to parents/family members/carers, and also members of the multidisciplinary team / external stakeholders (such as education staff).
- Actively contribute to transition and discharge planning.
- Utilise exceptional communication skills and have the ability to build rapport with children, reassuring / supporting anxious families.
- Identify, document and report any safeguarding concerns, as per Trust policy.
- Refer to other professionals/agencies for further assessment, advice or information as necessary.
- Contribute to the statutory Education, Health and Care Plan (EHCP) processes.
- Ensure privacy, dignity and individualised needs are met, demonstrating consideration of reasonable adjustments where appropriate.
- Be open, honest and participate in reflective learning, continuing professional development and revalidation in line with professional requirements.
We operate from three main hospitals – Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal – as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant‑led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non‑life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day‑case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our Community Service for adults is provided in people's homes, community centres, clinics, GP Practices, community hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
Our ICNT team provides services to the Lancaster, Morecambe and Carnforth areas only. We work across a number of community venues but the CCN team are currently based within the RLI.

Person Specification Qualifications- Registered nurse on NMC register
- Educated to degree level
- Experience of working with children
- Experience of working in the community

- Ongoing CPD
- Competence/experience of a range of relevant clinical skills
- Autonomous decision-making / prioritisation of workload / role limitations
- EMIS experience
