What you will do
  • Conduct thorough assessments of children with disabilities and their families to identify and address their needs.
  • Develop and implement personalised support plans in collaboration with families and multi‑agency partners.
  • Carry out safeguarding interventions and regular reviews to ensure the wellbeing of the children.
  • Support the transition of children and young people into adulthood, working closely with Young Adults Teams.
  • Engage with partners, including health professionals, special schools, and families, to deliver holistic services.
