Detailed OverviewReporting to the Manager, Clinical Operations, and as a member of the community-based interdisciplinary care team, the Occupational Therapist provides occupational therapy services to older and/or geriatric adults with chronic conditions. The role includes education to clients, caregivers and staff regarding rehabilitation interventions and advocacy for clients with community agencies and funding sources. Care is family centered and emphasizes the promotion, maintenance and restoration of health based on the chronic disease management model.
The position participates in quality improvement activities such as program authorization input and special/research projects related to older adult and/or geriatric clients.
- Provides occupational therapy assessment, consultation and interventions for older and/or geriatric adults to maximize physical function and promote independence and safety, determines appropriate use of recognized assessment tools, conducts assessments, prescribes and recommends specialized aids and equipment for ADL.
- Collaborates with interdisciplinary team members, client and caregivers to develop individual treatment goals, provides treatment and/or teaching, identifies resources required, and initiates requests for external funding as needed; provides verbal and written reports to the team.
- Evaluates the effectiveness of clinical care and education provided; supplies input for improvements to the team.
- Provides education and training for clients/family/caregivers and staff on independent living skills, occupational performance, use of adaptations, body mechanics, lifts and transfers; develops resource materials as required.
- Provides geriatric clinical expertise, consultation and leadership in evidence-based practice; evaluates practice and recommends changes; serves as a client advocate liaising with funding agencies and community resources.
- Maintains knowledge of evidence-based Occupational Therapy and geriatric practice by reviewing literature, consulting with professionals and attending education programs; sets measurable performance goals with the manager.
- Assists with research and special projects; collects data and provides input for analysis.
- Completes required reports, records and workload statistics; maintains files in accordance with standards.
- Assists with the development and monitoring of policies, procedures and standards; identifies staff learning needs and provides orientation; assists with student placements as assigned.
- Assists in the development and implementation of operational plans and mission for the Langley Specialized Seniors Team.
- Performs other related duties as assigned.
Graduation from an approved school of Occupational Therapy. Two (2) years of recent, clinical related experience with older and/or geriatric clients or an equivalent combination of education, training and experience.
Current full registration with the College of Health and Care Professionals of BC and eligible for membership in the BC Society of Occupational Therapists and/or Canadian Association of Occupational Therapists.
Valid BC Driver’s Licence and access to a personal vehicle for business related purposes.
Skills and Abilities- Demonstrated knowledge of current developments in Occupational Therapy relevant to older and/or geriatric adults.
