Position Title:
Community Outreach/Program Support Worker – Langley Hospice Society
Reports to:
Langley Hospice Society Executive Director, or designate.
Functional reporting:
Palliative Support Manager/Bereavement Support Manager
Position Type: Full time/On-site (37.5 hours per week), Wage: $30/hour, plus benefits
Position Summary:
Reporting to the Executive Director or designate, with a functional reporting relationship to our Program Managers, the Community Outreach/Program Support worker in collaboration with our palliative and bereavement program team, is responsible for the development and delivery of client and community-centred support programs and services, through the Langley Hospice Society.
They will assist with the provision of one-to-one and group palliative and bereavement support programs within the community and through our supportive program centre. They will provide ongoing support for fellow program staff and LHS volunteers in staff-led and volunteer-supported programming environments in the community, in hospice, and through our supportive program centre. They will assist with the delivery of community workshops and training, and connect regularly with community members and organizations to assist with information and access to palliative and bereavement services and supports, when they need them most.
As an integral part of the Langley Hospice Society’s Program Delivery Team, they will work collaboratively to ensure that program delivery is accessible and responsive to the needs of palliative and bereavement clients from the community and is consistent with the mission, vision and values of the Langley Hospice Society.
Typical Duties and Responsibilities - The successful candidate will:
Coordination & Communication
● Assist with the development of a vibrant community outreach program, by ensuring that information
and resources to access palliative and bereavement support and services are accessible for clients and the community.
● Support and collaborate with program staff to ensure timely access to programs and services
● Support and collaborate with volunteers as appropriate.
● Maintain regular communication with community organizations, clients, volunteers, program managers and fellow staff.
● Refer clients to other community programs and services, as appropriate.
● Coordinate special events and activities as needed.
● Participate in weekly program check-ins, and monthly program and all-staff meetings.
● Respond to enquiries by phone, email, and in person in a timely and professional manner.
Facilitation and Training:
● Work with the existing curriculum, and update, develop and deliver relevant programs and
workshops to prospective volunteers, clients, colleagues, and community members, as appropriate.
● Coordinate and/or facilitate other training opportunities for volunteers, fellow staff and the
community.
● Facilitate information and educational sessions for community groups, and individuals as
appropriate.
Volunteer Support
● Support and work collaboratively with trained LHS volunteers who assist the program team in
providing support and service, in community, in hospice residence and through our supportive program centre.
● Ensure that volunteers are trained, orientated and supported in their roles.
● Coordinate and schedule volunteers to meet program needs with collaboration and/or communication with the program managers and fellow staff, as appropriate.
Palliative & Bereavement Program Support
● Will provide information and resources for individuals and organizations in the community who are
looking to understand the palliative approach to care, and how to access support and services as
they approach end-of-life.
● Deliver client and community-centred one-to-one support and/or group support for individuals who
have experienced the death of a significant person in their lives, no matter the timeline or
circumstances around the death.
● Facilitate and foster engagement of trained volunteers within the community, hospice residence and
supportive program centre to increase access to LHS support programs and services.
