Fraser Health is looking for a dedicated Licensed Practical Nurse to join its Community Health Teams. In this role, you will provide assessments, health coaching, and direct care aimed at enabling clients to thrive will collaborate with an interprofessional team to optimize client transitions and advocate for their healthcare decisions, ensuring culturally safe and inclusive practices.
Key Responsibilities:
• Develop therapeutic relationships through effective communication
• Manage individualized care plans for clients with chronic conditions
• Conduct health assessments and track client progress
• Coordinate care with primary health providers and community services
• Participate in quality improvement and education initiatives
Requirements:
• Current BC Registration as a Licensed Practical Nurse
• One year of clinical experience with complex patient care
• Understanding of chronic health conditions and community resources
• Valid BC Driver's license and personal vehicle
• Strong organizational and teaching skills
