Position: Health & Wellness Manager (LPN)
Amenida Seniors Community is seeking an experienced and compassionate Health & Wellness Manager (LPN) to provide clinical leadership and support resident care at our Surrey community.
This leadership role works closely with site Operations, the Assisted Living team, physicians, pharmacy partners, residents, and families to ensure safe, resident-centred care and strong regulatory compliance.
The successful candidate will provide leadership in resident assessments and Personal Service Plans (PSPs), medication management oversight, clinical documentation, incident management, staff education and competency, family communication, and compliance with BC assisted living requirements.

Key Responsibilities   Lead resident assessments, reassessments, and development and review of Personal Service Plans (PSPs).
Provide oversight of medication management practices and support safe medication administration processes.
Coordinate with physicians, pharmacy partners, and other healthcare professionals regarding resident health and care needs.
Lead clinical incident follow-up, investigation, documentation, and required reporting.
Support compliance with Assisted Living Registry, Fraser Health, and applicable regulatory requirements.
Provide education, competency support, coaching, and guidance to Assisted Living staff.
Monitor changes in residents' health, cognition, behaviour, and care needs and coordinate appropriate interventions.
Support hospital transfers, discharges, and safe transitions back to the community.
Communicate effectively and compassionately with residents and families regarding care needs and changes.
Maintain accurate and timely clinical documentation and ensure appropriate follow-up.
Work collaboratively with site Operations and leadership to identify risks, strengthen processes, and continuously improve resident safety and quality of care.
Qualifications    Current practising registration as a Licensed Practical Nurse (LPN) with the British Columbia College of Nurses and Midwives (BCCNM) is required.
Experience in seniors care, assisted living, long-term care, or a related healthcare environment.
Previous leadership, supervisory, or clinical coordination experience preferred.
Knowledge of resident assessment, care planning, medication management, incident reporting, and regulatory compliance.
Strong leadership, communication, problem-solving, and documentation skills.
Ability to work collaboratively with residents, families, staff, physicians, pharmacy partners, and operational leadership.
Experience supporting residents living with dementia and responsive behaviours is an asset.
