Position: LPN with Resident-Centered Care Focus

Reporting to the Director of Care, you will coordinate and administer quality care for residents with a focus on their individual needs. The position requires you to implement care plans, assess changing health conditions, and engage with a team committed to ensuring the rights and well-being of every resident.

Key Responsibilities:

• Develop and assess individualized care strategies

• Provide medication and treatment within scope

• Document and report health changes promptly

• Conduct interdisciplinary care conferences

• Coach and mentor new staff members

Requirements:

• Current LPN license with the BCCNM

• Certification in Occupational First Aid Level 1

• Completion of RAI competency testing required

• Prior long-term care experience is beneficial

• Skills to manage physical tasks and maintain a clean environment
