Your role focuses on enhancing person-centered care.
In this position, you will provide therapeutic counseling, manage case documentation, and collaborate with an interdisciplinary team for resident care planning. Your expertise will help navigate family needs with legal, financial, and emotional support. Engaging in quality improvement initiatives, you will contribute to the development of social work services aligned with Langley’s goals.
Key Responsibilities:
• Provide therapeutic counseling for residents and families
• Collaborate with care teams on resident care plans
• Maintain accurate and detailed resident health records
• Coordinate with external resources for resident support
• Educate families about rights and community resources
Requirements:
• Bachelor’s Degree in Social Work
• At least two years of clinical experience
• Registration with the BC Social Workers Board
• Familiarity with electronic health records
• Understanding of elder care regulations
