This role includes conducting assessments, therapeutic counseling, and ongoing monitoring of residents' psychosocial well-being. You will partner with families and interdisciplinary teams to ensure high-quality care and support resources. Participation in quality initiatives and ensuring compliance with care standards will be key to your contributions.
Key Responsibilities:
• Conduct comprehensive psychosocial assessments
• Facilitate communication in Resident Care Conferences
• Document and maintain resident care records
• Provide crisis intervention and emotional support
• Liaise with external agencies for resident services
Requirements:
• Bachelor’s Degree in Social Work
• Minimum of two years in Long Term Care settings
• Active registration with the BC Social Workers Board
• Proficient in Microsoft Office and health systems
• Knowledge of relevant healthcare legislation
