About the job:

The Transportation Scheduler Dispatcher under the supervision of the Transportation Manager, the Scheduler is responsible for the timely scheduling of transportation for Senior Community Care (SCC) participants to and from the Day Center and/or appointments. The Scheduler is also responsible for scheduling outside medical appointments for the Senior Community Care participants and notification of appointments with participants, their family and home care providers.

Responsible for communicating any safety or scheduling issues to the Transportation Manager.

Required Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.   • High school diploma or equivalent required;
  • Have a minimum of one years’ experience in working with the frail elderly.
  • Must pass a background check and drug screen.
  • Valid Driver’s License in state of hire.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Skilled in establishing and maintaining effective working relationships with participants, caregivers, drivers and co-workers.
Essentials:

1. Familiar with Pace Care Scheduling Program to be able to gather information needed for timely and
efficient individual transportation and home care schedules.

2. Considers the needs of the participant and requirements of the PACE Center. Communicates these
changes to Transportation Manager.

3. Develops schedules to limit employee overtime hours as much as possible.

4. Schedules Transportation for participants to and from the Day Center, medical/specialist
appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care. Also
Schedules Transportation for last minute emergencies that arise.

5. Assists in scheduling vehicle maintenance and equipment to operate within manufactures
recommendations.

6. Functions as a member of the Interdisciplinary Team when appropriate.

7. Communicates with Transportation Manager daily to be updated on participant conditions/ changes,
plan of care changes, or Interdisciplinary Team requests
