Position Overview
Position Objective
Provides psychosocial assessment, individual, family, and group therapy to a caseload of patients. Conducts assessments, plans for and implements identified services for patients. Works with patients and families on safe discharge plans. Collaborates with providers and the interdisciplinary team on care plans.
Responsibilities- Provides comprehensive psychosocial assessments for patients on caseload.
- Creates a therapeutic framework for effective problem solving with patients and families.
- Responds to patient/family needs related to mental health issues.
- Provides transition of care strategies for complex situations such as substance abuse, psychiatric, dual diagnosis placement, ventilator-dependent patients, inpatient dialysis, uninsured patients requiring skilled nursing facility placement, guardianship, long‑term/custodial placement.
- Identifies community services and resources and facilitates linkage with those services.
- Communicates progress and status to the interdisciplinary team, making recommendations and revising plans based on ongoing evaluation and collaboration.
- Evaluates patient/family progress toward established goals and participates in rounds and care conferences as needed.
- Integrates medical record and nursing information in assessment and interventions.
- Ensures timely, appropriate, and accurate evaluation and reporting of suspected child/elder abuse and/or neglect. Assesses for domestic violence issues and involves the domestic violence program coordinator as indicated.
- Documents interventions in the patient's medical record following department standards.
- Participates in process improvement activities in the Mental Health and Substance Use Quality Council.
- Proactively promotes social work and other psychosocial services to the care team and recommends services to patients who may benefit.
- Facilitates patient/family support groups and assists in mentoring new professional staff and student interns.
- Master’s of Social Work degree (MSW).
- Minimum of 1 year of work in a mental health or substance use program.
- Two years of oncology social work experience.
- Two years of experience rendering clinical social work services in a health care environment.
- Current license as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker – Certified (LCSW‑C).

Pay Range$35—$50 USD
