Acts is currently seeking qualified candidates for our Social Worker position.

In this role, you will be responsible for providing medically related social services to support the well‑being of our residents and their families. Participates in the admission process in collaboration with the Nursing Home Administrator and Director of Nursing. Assesses residents’ psychosocial needs and delivers individualized support services, as needed.
Requirements
The ideal candidate will meet the following requirements:
- Bachelor's degree in social services or a related field
- Minimum of two years' experience in a long‑term care setting
- General knowledge of social services required in a healthcare setting

Pay Range: $25.75 - $33.05 / hour. Starting rate will vary based on skills and experience.
