LPN Supervisor for Personal Care

12 Hr. Shift - Two (2) Days Per Week: 7:00 pm – 7:00 am
Every Other Weekend: 7:00 pm – 7:00 am
We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) Supervisor to lead and oversee nursing staff within our Personal Care Community. This role offers an excellent opportunity to contribute to high‑quality patient care while providing leadership and support to our Med Tech’s and PC Aides. The ideal candidate will possess strong clinical skills, leadership abilities, and a commitment to fostering a positive and compliant work environment.
Key Responsibilities- Supervise and coordinate the daily activities of licensed practical nurses, PC Aides and MED TECHS.
- Ensure compliance with all healthcare regulations, policies, and procedures.
- Provide mentorship, training, and performance evaluations for nursing staff.
- Assist in developing and implementing patient care plans.
- Monitor patient care quality and address any issues or concerns promptly.
- Collaborate with physicians, healthcare teams, and administrative staff to optimize patient outcomes.
- Maintain accurate documentation and ensure confidentiality of patient information.
- Participate in staffing schedules and manage resource allocation effectively.
To provide professional direct resident nursing care and, in conjunction with the Charge Nurse, to supervise daily nursing activities performed by Nursing Assistants. To promote and maintain a positive physical, social, and psychological environment for residents in the skilled nursing facility. To perform these duties in accordance with applicable federal, state, and local regulations, as directed by the Charge Nurse, and in line with the mission of Elm Terrace Gardens to promote the health and well‑being of individual residents.
Duties- Ability to sit, walk, reach, bend, lift, grasp, push, pull, and have fine hand coordination.
- Ability to read and write in English.
- Ability to communicate with staff, physicians, consultants, residents, families, visitors, and vendors.
- Ability to distinguish smells, tastes, and temperatures.
- Ability to hear and respond to call bells, alarms, and emergency signals.
- Ability to remain calm under stress.
