At Twin Oaks Health & Rehab, we are dedicated to providing exceptional care to our residents and are seeking a talented Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping our patients achieve their optimal level of physical function and independence.

As a Physical Therapist at Twin Oaks Health & Rehab, you will be responsible for:

• Evaluating and treating patients with various physical disabilities and injuries
• Developing and implementing individualized treatment plans to achieve patient goals
• Providing education and guidance to patients, families, and caregivers on proper therapeutic exercises and techniques
• Working collaboratively with other healthcare professionals to ensure comprehensive care
• Maintaining accurate and detailed records of patient progress and treatment plans

• Assess patients' physical abilities and develop treatment plans to improve function and mobility
• Provide individualized therapy sessions, including exercises, stretches, and other interventions
• Monitor patient progress and make adjustments to treatment plans as needed
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care
• Maintain accurate and detailed records of patient progress and treatment plans
