• Provides physical therapy services according to a written individualized plan of care.
• Assesses and evaluates therapeutic/rehabilitative/functional status, and participates in development of total plan of care.
• Assesses home environment.
• Evaluates muscle strength, mobility, gait, ROM, and transfer capabilities to improve or minimize residual physician disabilities.
• Provides physician or advanced practice practitioner prescribed physical therapy.
• Educates patients on equipment required to increase functional independence.

• Possesses a degree from an accredited Masters or doctoral program as a Physical Therapist.
• 1-year clinical experience as a Physical Therapist preferred.
• Interpersonal skills and ability to communicate professionally with staff, patients, and caregivers.
• Knowledge of medical terminology.
• Problem-solving, conflict resolution, organizational skills, and attention to detail.
• Strong verbal and written communication skills.
• Ability to handle multiple priorities and documentation requirements.
• Reliable transportation and valid automobile insurance coverage.
• Physical capability to sit, stand, bend, lift, and move intermittently, and lift at least 35 lbs.

• Home Health experience preferred.
• Familiarity with Home Care Home Base (HCHB).
