Pay: $27.00-$31.40
Hours: Monday‑Friday 8:00 AM‑4:30 PM
The LPN Med Room, under the direction and supervision of the Clinical Care Manager and in coordination with the Medication Technician, is responsible for the effective management and provision of participant medications provided via the PACE medication department. Administers medications, implements primary care orders, and evaluates participant needs physically and/or by telephone. Provides education to Senior Community Care participants and caregivers regarding plan of care and medications.
Communicates with participant’s primary care givers, physicians, clinic staff and interdisciplinary team.
Assume responsibility for processing medication orders for all participants, coordinating schedules, reconciling transitions, communicating with providers and pharmacies, and maintaining on‑site medication stocks.
Required Qualifications- Graduate of a professional nursing school
- Current Practical Nursing License (L.P.N.)
- Cardiopulmonary Resuscitation (CPR) certification
- Minimum one year experience working with the frail or elderly population
- Valid driver’s license, proof of insurance and means of transportation
- Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact
- Intravenous Venous (IV) Certification
- Process medication orders for all participants through direct input or processing of medication orders input by staff provider, ensuring complete, accurate, and legal prescriptions.
- Review notes from outside providers and identify and process medication orders with approval of PACE provider.
- Coordinate participant medication schedules, including implementation of new orders, mid‑cycle changes, discontinuations, and reconciliations through transition processes as prescribed by policy.
- Conduct medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) for all participants in accordance with organization procedures and facilitate any required changes.
- Communicate with and between PACE providers, community‑based providers, and the pharmacy regarding medication orders, clarifications, and pharmacy recommendations.
- Coordinate with local pharmacies to obtain urgent medications that are not available through on‑site dispensing, including transmission of medication order, demographics, and insurance billing information.
- Facilitate on‑site medication dispenses through the system provided through the pharmacy and perform periodic inventory and maintenance of on‑site medication stock.
