• Provide specialized nursing care and initial and ongoing comprehensive assessments of patients.
• Conduct OASIS assessments, develop and modify care plans, and initiate diagnostic, preventative, and rehabilitative procedures.
• Coordinate care with multidisciplinary teams, attend client care conferences, and supervise home health aides and LPNs.
• Maintain accurate and timely documentation, including client progress records.
• Participate in in-service programs and quality improvement initiatives; ensure compliance with all policies and procedures.
• Plan for client discharge from services.

• Associate’s degree in Nursing from an accredited program.
• Registered Nurse license in Michigan.
• Minimum 1 year of RN experience preferred.
• Knowledge in caring for adult and geriatric homebound populations.
• Computer proficiency for electronic medical record use.
• Valid driver’s license and reliable transportation.
• Current auto insurance.

$75,000 – $85,000 USD
