Pay: $27.00‑$31.40 per hour
Schedule: Monday‑Friday 8:00 AM–4:30 PM
Position Overview: Under the direction and supervision of the Clinical Care Manager and in coordination with the Medication Technician, the LPN is responsible for the effective management and provision of participant medications via the PACE medication department. Responsibilities include administering medications, implementing primary care orders, evaluating participant needs physically and/or by telephone, providing education to participants and caregivers regarding plan of care and medications, and communicating with prescribers, physicians, clinic staff and the interdisciplinary team.
Required Qualifications- Graduate of a school of professional nursing
- Current Practical Nurse license (L.P.N.)
- Cardio Pulmonary Resuscitation (CPR) certification
- Minimum of one year’s experience with the frail or elderly population
- Valid driver’s license, proof of insurance and means of transportation
- Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact
- Intravenous Venous (IV) Certification
- Processing medication orders and ensuring complete, accurate, legal prescriptions are received by dispensing pharmacy
- Reviewing outside provider notes and processing medication orders with approval of PACE provider
- Coordinating medication schedules, new orders, mid‑cycle changes, discontinuations, and reconciliations per policy
- Managing medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge)
- Communicating with PACE providers, community-based providers, and pharmacy regarding orders and recommendations
- Coordinating with local pharmacies for urgent medications not available on‑site
- Facilitating on‑site medication dispenses and maintaining on‑site medication inventory
