Nurse Practitioner, Primary Care & Population Health

Oak Street Health is hiring a Nurse Practitioner in Lansing, MI to deliver value-based primary care to adults on Medicare. You will see patients independently within a collaborative care team and help keep patients happy, healthy, and out of the hospital.

You'll work with physicians, nurse practitioners, PAs, and care managers in outpatient clinics, performing wellness visits, care coordination, and population health initiatives.
