We are looking for a compassionate OT to join our growing team! The right candidate has a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide medical support to our clients in their homes.

• Provides Occupational therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to: assessing and evaluating therapeutic/rehabilitative/functional status, participating in the development of the total plan of care, and evaluating the home environment.
• Assesses for muscle strength, mobility, ROM—potential for rehab.
• Directs Occupational therapy treatment.
• Instructs patients and families/caregivers in the use and care of therapeutic appliances.
• Determines priority needs for Occupational therapy.
• Reports to the physician on the patient’s reaction to treatment or changes in condition.
• Initiates the Occupational therapy program and instructs other personnel and/or family/caregiver members in phases of occupational therapy, including participating in case conferences to align goals for the patient’s program.
• May train the patient in the use of prosthetic devices.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on goal attainment.
• Participates in discharge planning for patients.
• Provides occupational therapy consultation to home families/caregivers when indicated.
• Provides in-service education programs for nursing organization personnel as needed.
• Participates in the peer consultation process.
• Supervises occupational therapy assistants according to organization policy and state regulations.

• Possesses a degree from an accredited occupational therapy program.
• Licensed to practice as an Occupational Therapist within the state.
• Two years of experience as an Occupational Therapist; community/home health experience preferred.
• Good verbal and written communication and organizational skills.
• Current CPR certification.
