An exciting opportunity has arisen for a highly motivated, enthusiastic and innovative Specialist Physiotherapist to join the established Multidisciplinary Hospital at Home (H@H) service in Forth Valley. The Physiotherapy team is expanding to support additional Respiratory H@H beds.

As a Specialist Physiotherapist you will work in partnership with colleagues to facilitate early supported discharge of patients from the acute beds and prevent hospital admissions of acutely unwell patients in the community. These patients are managed in the H@H virtual ward.

The H@H Team offers a varied clinical caseload with excellent peer support and training opportunities. You will be expected to participate in quality improvement, evidence based practice and in‑service training. The role will involve the clinical supervision and appraisals of more junior staff, as well as undergraduate / pre‑reg students.

We are looking for someone who has varied clinical experience across community and acute specialisms with an interest in respiratory, frailty and acutely unwell patients. The applicant should possess the ability to prioritise fluctuating workloads and be flexible in a busy environment.

The successful candidate will be supported in their professional development of enhanced clinical knowledge and clinical decision‑making skills.

The service operates extended hours over 7 days and covers the whole of Forth Valley from their base in Falkirk Community Hospital. Applicants must be able to travel across the Forth Valley area.

Responsibilities
  • Work in partnership with multidisciplinary colleagues to facilitate early supported discharge of patients from acute beds.
  • Prevent hospital admissions for acutely unwell patients in the community and manage them within the H@H virtual ward.
  • Participate in quality improvement, evidence based practice and in‑service training.
  • Provide clinical supervision and appraisal for junior staff and undergraduate/pre‑reg students.
  • Supervise extended‑hours coverage over 7 days across Forth Valley, travelling as required.
Qualifications
  • Varied clinical experience across community and acute specialisms, with an interest in respiratory, frailty and acutely unwell patients.
  • Ability to prioritise fluctuating workloads and remain flexible in a busy environment.
  • Strong communication, decision‑making and patient‑centred care skills.
Professional Development & Working Hours

The successful candidate will be supported in professional development, enhanced clinical knowledge and decision‑making skills. As from 1/4/26, the Agenda for Change full‑time working week will be reducing from 37 to 36 hours per week. Part‑time hours will be reduced pro‑rata with a corresponding hourly wage increase to maintain overall pay.
