JOB PURPOSE

Endeavors Workforce Wellness Program (WWP) is seeking an Intake Coordinator to support U.S. Customs and Border Protection (CBP) personnel and their families. The Intake Coordinator is responsible for enrolling, facilitating, and implementing comprehensive programs and activities aligned with Endeavor's six principles of Wellness. Additionally, they conduct initial eligibility screenings for individuals and family members seeking program enrollment. This position is dedicated to ensuring access to health and wellness services for all enrolled individuals in the holistic health and wellness program.

ESSENTIAL

JOB RESPONSIBILITIES
  • Establishes initial program intake file for clients and their families entering the program.
  • Promotes wellness initiatives; answers questions and collects feedback from participants.
  • Collaborates with Program Manager and Case Manager to assess and analyze client’s general wellness and clinical health needs.
  • Collaborates with external entities for support, events, and other opportunities.
  • Supports planning and coordination of the program and its activities.
  • Assigns clients to wellness, fitness, clinical, other services and tracks client progress through the program.
  • Prepares and keeps data, files, reports, drafts and accurate reporting of intakes and referral activities.
  • Communicates with team members to ensure maximum efficiency.
  • Manages daily assignments and ensures deadlines are met and work is completed correctly.
  • Monitors the progress of the client concurrent with the program and adjusts as necessary to ensure the successful completion of intakes.
  • Provides supplemental assistance of programs such as classes, support groups, life skills classes, and other activities to support client needs.
  • Assists Program Manager and Case Manager in monitoring program referrals, intakes and wait lists.
  • Maintains accurate, confidential records and reports as required to track and assess client participation.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS

EDUCATION: Bachelor’s Degree in related field preferred, or equivalent related experience.

EXPERIENCE:

  • Minimum two years of Case Management experience.
  • Proven ability to work cohesively as part of an interdisciplinary team.
  • Experience working with first responders, law enforcement personnel, or high-performance occupations is strongly preferred.
  • Must demonstrate leadership capacity and ability to foster team building and a supportive work environment.
