Provides mental health counseling to City residents. Schedules appointments, maintains documentation for counseling sessions, and prepares treatment plan to ensure positive patient outcomes.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIESThe intent of this job description is to provide a representative summary of the major duties and responsibilities performed by employees in this job. Employees may be requested to perform job-related tasks other than those specifically presented in this description.
- Provide individual, family, and group counseling using evidence-based and trauma‑informed
therapeutic modalities. - Conduct comprehensive biopsychosocial assessments, diagnostic evaluations, and treatment
planning in alignment with professional standards and agency protocols. - Develop individualized treatment plans with measurable goals, regularly review progress, and
adjust interventions to support client recovery and wellbeing. - Deliver crisis intervention, risk assessments, safety planning, and coordination of emergency
services as needed. - Facilitate psychoeducation sessions on mental health conditions, coping skills, emotion
regulation, stress management, and related topics. - Maintain a culturally responsive and client‑centered approach, ensuring services are equitable and accessible for diverse populations.
- Coordinate care with internal team members (case managers, telepsychiatry providers, social workers, health educators) to support integrated behavioral health services.
- Collaborate with external partners such as schools, hospitals, social service agencies, community organizations, and referring providers.
- Participate in interdisciplinary case reviews, staffing meetings, and care‑coordination activities to support continuity of care.
- Provide referrals to community resources including housing, food assistance, medical care,
substance use treatment, and supportive services. - Maintain accurate, timely, and compliant clinical documentation, including progress notes,
assessments, treatment plans, discharge summaries, and required reporting. - Adhere to confidentiality laws, ethical codes of conduct, and state/federal regulations including HIPAA.
- Utilize electronic health records {EHR) or program‑specific systems to manage documentation, billing components, and data tracking.
- Collect and report program metrics {PHQ-9, GAD-7, Quality of Life, satisfaction surveys,
outcomes data) to support quality improvement and grant reporting. - Support the delivery of community mental health initiatives including outreach events,
educational workshops, and prevention activities. - Participate in program development efforts such as curriculum design, group facilitation models, and innovative approaches.
- Provide mentorship or supervision to interns or trainees as applicable.
- Represent the program professionally in community settings and contribute to destigmatizing conversations about mental health.
- Engage in continuing education, supervision, and professional development to maintain
licensure and enhance clinical skills. - Uphold organizational values related to diversity, equity, inclusion, compassion, and trauma‑informed care.
- Support quality improvement initiatives and help identify opportunities to strengthen service delivery.
- Perform other duties as assigned to support the mission and expansion of community mental health services.
- Will be required to drive a city vehicle for city business use.

- Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
