Devoted Health is expanding its Medical Transitions of Care program. APPs (APRNs and PAs) will lead post-discharge visits, manage complex geriatrics, and collaborate with care coordinators, social workers, and physicians to prevent readmissions.
Roughly 70% of time is initial post-discharge visits with 30% follow-ups; strong clinical judgment and long-term care planning are essential. Telehealth and in-person visits support a comprehensive, wrap-around approach.
