Delivery of case management services to an adult population related to substance use and mental health. Services include outreach, identification, screening, assessment and referral to community sources to address their health, social and economic needs. Extensive liaison work with referral sources. Responsible for tracking of assigned clients and maintenance of appropriate records in a timely and accurate manner. Case manager is also responsible for monitoring and expanding referral networks with agencies and organizations.

- Bachelor’s degree in a directly related field plus four years of direct client service experience in mental health or substance abuse.
- Master’s degree in counseling or a directly related field with two years of direct client service experience in mental health or substance abuse, licensed or pursuing licensure preferred.
- Screening of clients to determine appropriate level of care.
- Provide appropriate Brief Interventions, using principles of Motivational Interviewing, to clients to motivate and ensure compliance to screening recommendations.
- Establish educational groups for clients to provide appropriate information regarding substance abuse.
- Demonstrates knowledge of accepted principles of client record management.
- Protects client rights to privacy and confidentiality in the preparation and handling of records, especially in relation to the communication of client information with third parties.
- Prepares accurate, timely, and concise client records that are consistent with agency standards and comply with all applicable confidentiality rights of adolescents.
- Adheres to the agency code of ethics and business practices, professional codes of ethics, and all applicable state and federal laws regarding the treatment of substance use disorders.
- Utilizes supervision, self‑evaluation, and consultation to enhance self‑awareness and counseling skills and professional development.
- Regularly participates in continuing education.
- Understands a variety of models and theories of addiction and other problems related to substance abuse and uses this knowledge to plan intervention strategies with clients.
- Recognizes the social, political, economic and cultural context within which addiction and substance abuse exist.
- Identifies the behavioral, psychological, physical health and social effects of psychoactive substances on the client and family members.
- Recognizes the potential for substance abuse disorders to mimic a variety of medical and psychological disorders and the potential for medical and psychological disorders to co‑exist with addiction and substance abuse.
