• Evaluates psychosocial aspects of patients and families, refers and assists in the referral process to community agencies.

• Driver’s License – State of Florida (required within 30 days of hire)
• Master’s – Social Work or Master’s – Counseling
• 1 year of Social Work experience
• Preferred 1 year of Home Care experience
