Join as a Home Health Field Physical Therapist at Bay Care Home Care, one of Tampa Bay’s largest employers.
Bay Care Home Care offers a flexible 8:00 a.m.–5:00 p.m. Monday‑Friday schedule with occasional weekend work, full‑time exempt status, and benefits such as health, dental, vision, paid time off, tuition reimbursement, 401(k) match, and performance bonuses. This position serves patients in Largo, St. Petersburg, and surrounding Pinellas Park zip codes.

- Evaluate and develop treatment programs for individual patients based on their needs and the principles and practices of physical therapy.
- Provide appropriate clinical direction for Physical Therapy Assistants.
- Plan, implement, manage and evaluate the provision of professional and ancillary home health services to ensure all patient needs are met and quality care is provided in accordance with federal, state, JCAHO and program guidelines.
- Manage the activities of the multidisciplinary team providing care to the patient.
- Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
- Develop, implement, and carry out a discharge plan in collaboration with the patient, family/caregiver, and members of the health care team.
- Interpret agency policy for patients/families.
- Review patient visits weekly and provide a plan to the Home Care Supervisor and scheduler, including anticipated weekend visits.
- Inform scheduler of any schedule changes and communicate caseload and patient care issues to the Home Care Supervisor.
- Bachelor’s Degree in Physical Therapy
- Physical Therapist license in the State of Florida
- Minimum 1 year of physical therapy experience
- BLS (Basic Life Support) certification
- Driver’s license (Florida driver’s license required within 30 days of hire)
- Preferred:
Master’s Degree in Physical Therapy
