Weekend Nurse Practitioner (Skilled Nursing) – Aviata Health Group

Location:

Largo, Florida

• Act as the Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday
• Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends
• Work with the entire facility’s patient population – short and long-term residents, assisted by the nursing staff and guidance from the medical directors and attending physicians
• Evaluate new-admission visits
• Coordinate discharge visits
• Perform evaluations for patients exhibiting changes in condition
• Provide comprehensive care, including but not limited to:
  • Advanced Care Planning
  • Comprehensive health assessments, differential diagnoses, and prescribe treatments under the protocol for acute and chronic illness and disease in direct management of patient care
  • Prescribe all necessary medications and treatments
  • Perform comprehensive physical examinations of patients
  • Develop and implement patient management policies and procedures
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with attending physicians and/or specialists regarding patient care

• Advanced Practice Licensure (APN) with a Florida license
• Active license and DEA number
• Previous experience in nursing or other medical fields
• Ability to build rapport with patients and their loved ones
• Ability to thrive in a fast‑paced environment
• Excellent written and verbal communication skills
• Strong leadership experience with emphasis on teamwork in a healthcare setting
• Master’s degree required
