Position Details
  • Position: LPN / LVN
  • Shift Type: Full Time
  • Hours: Night (10pm-6am) (every other weekend required)
  • Wage: $24–$28.50 per hour based on experience
Summary of Role

The Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) plays an integral role in the success of our team, our community and creating a place our residents are proud to call home. The primary purpose of this position is to provide high quality, holistic nursing care in accordance with nursing practice standards and health care regulations which govern the practice of professional nursing.

Lead or direct licensed and non-professional staff in the delivery of direct resident care and support functions.

Responsibilities Nursing Care
  • Conduct daily nursing functions in accordance with rules, regulations, and guidelines governing long‑term care facilities.
  • Ensure nursing personnel assigned to you comply with written policies, procedures, and the safety program.
  • Admit, transfer, and discharge residents with documentation of care and service.
  • Participate in the orientation of new residents and family members to the facility.
  • Make rounds with healthcare provider, Hospice staff, and Home Health staff as needed.
  • Utilize nursing personnel qualifications according to resident care plans and provider orders.
Communication & Documentation
  • Complete daily clinical and environmental rounds, evaluate resident care, and report significant events.
  • Meet and resolve issues with residents, family members, and report to supervisor.
  • Participate in facility surveys (inspections) as requested.
  • Assist with clinical programs such as dementia, wound care, quality assurance, infection control, admissions.
  • Complete assessments, develop interim care plans, and update care plans.
  • Communicate with healthcare provider to review treatment plans, complete orders, progress notes, etc., per established policies.
  • Review medical records for treatments, medication orders, diets, labs.
  • Arrange diagnostic and therapeutic services per provider orders.
  • Communicate policies and procedures to personnel, residents, visitors.
  • Report changes in resident condition to nursing management.
  • Complete accident/incident reports and medical record documentation.
  • Provide resident and family information for care conferences or meetings.
  • Notify healthcare practitioner and family of changes in resident condition.
  • Assist in arranging transportation for residents and third‑party services.
Medication Administration
  • Prepare and administer medications ordered by the provider.
  • Coordinate with pharmacy to ensure medication availability.
  • Maintain adequate supply of floor‑stock medications, supplies, equipment.
  • Verify completeness and accuracy in medication administration records.
  • Dispose of drugs and narcotics per procedures.
Personnel
  • Supervise nursing unit to ensure adherence to nursing standards.
  • Work with Director of Nursing to resolve staff performance issues.
  • Report complaints and grievances to management and collaborate on action plans.
Safety & Sanitation
  • Ensure availability of protective equipment for staff.
  • Maintain nursing service areas, resident rooms, and treatment areas clean and sanitary.
  • Recommend equipment and supply needs to Director of Nursing.

    Maintain stock levels of medications, supplies, and equipment.
  • Perform other job‑related duties as assigned.
Knowledge & Critical Skills
  • Make independent decisions and follow instructions.
  • Interact tactfully with personnel, residents, family, visitors, government agencies, and the public.
  • Work effectively with ill, disabled, elderly, or emotionally upset individuals.
  • Communicate effectively with supervisors, team members, prospects, residents, and families.
