The APRN provides exceptional medical care, conducting physical exams, ordering and interpreting diagnostic tests, crafting treatment plans, and coordinating care under physician supervision. This safety‑sensitive role involves direct patient care and medication administration.
Responsibilities- Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine.
- Provide care for patients with acute and chronic illness encountered in the older adult patient population.
- Accountably manage patient care and outcomes, seeking specialist consultation when needed and maintaining detailed responsibility for the patient’s care.
- Engage with hospitalists whenever a patient is in the hospital.
- Meet all clinical obligations of the medical practice:
Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc. - Assess, diagnose, treat, manage, educate, promote health, and provide care coordination for patients with acute and complex chronic health needs using evidence‑based medicine and practice guidelines.
- Follow all internal policies and procedures, completing operational tasks according to departmental guidelines.
- Maintain complete and detailed records electronically in the EMR, completing documentation within 48 hours of each patient’s visit.
- Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability.
- Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
- Interpret data and diagnostic results for appropriate courses of action including but not limited to blood gases, radiology, and EKG, in collaboration with the supervising physician.
- Exhibit compassionate, quality‑focused, and conscientious behavior.
- Establish and maintain effective working relationships with patients, families, and team members.
- Document continuing education per licensure and certification requirements.
- Ensure patients are seen every other visit by the Physician.
- Maintain patient confidentiality in accordance with policy, procedure, and HIPAA requirements.
- Enact proper chain of command for patient complaints.
- Perform additional duties as assigned.
- Graduate of an accredited school of nursing.
- Certified nurse practitioner with current state licensure.
- At least 1 year of professional nursing experience, including 1‑3 years as a primary‑care nurse practitioner.
- Prior experience working in an electronic medical records system.
- Provide 5‑Star service in accordance with Better Health Group’s mission.
- Skilled in traditional nursing disciplines of patient assessment and management.
- ACLS/BLS certification required.
- Demonstrated knowledge of appropriate patient care and the ability to handle emergency situations calmly and effectively.
- Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
- Strong written and verbal communication, teaching ability, and telephone skills.
- Working knowledge of Google Workspace (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint).}
- Full range of body motion, including handling and transferring patients.
- Requires standing, walking, pushing, bending, kneeling, and reaching at arm’s length and overhead for prolonged periods.
- Must be able to lift and move patients and medical equipment as needed, with a maximum lift of 15 lbs.
- Requires corrected vision and normal hearing.
- Ability to sit for extended periods and operate a motor vehicle.
