Position: Weekend Nurse Practitioner (Skilled Nursing)

Aviata Health Group is looking for a Nurse Practitioner with SNF experience

Largo, Florida

Major Responsibilities
  • Act as the Skilled Nursing Facility (SNF)’s leading provider of internal medicine services on Saturday and Sunday.
  • Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends.
  • Work with the entire facility’s patient population—short‑ and long‑term residents—supported by the facility’s nursing staff and under guidance from medical directors and attending physicians.
  • Evaluate new‑admission visits.
  • Coordinate discharge visits.
  • Conduct evaluations for patients exhibiting changes in condition.
  • Provide advanced care planning and comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.

Qualifications
  • Advanced Practice Licensure (APN) with a Florida license.
  • Active license and DEA number.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast‑paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.
