Aviata Health Group is looking for a Nurse Practitioner with SNF experience
Largo, Florida
Major Responsibilities- Act as the Skilled Nursing Facility (SNF)’s leading provider of internal medicine services on Saturday and Sunday.
- Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends.
- Work with the entire facility’s patient population—short‑ and long‑term residents—supported by the facility’s nursing staff and under guidance from medical directors and attending physicians.
- Evaluate new‑admission visits.
- Coordinate discharge visits.
- Conduct evaluations for patients exhibiting changes in condition.
- Provide advanced care planning and comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
- Prescribe all necessary medications and treatments.
- Perform comprehensive physical examinations of patients.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.
- Healthy work‑life balance with flexibility.
- Malpractice insurance and tail coverage for $1 MM/$3 MM coverage provided by Company.
- Revenue‑split compensation.
- Advanced Practice Licensure (APN) with a Florida license.
- Active license and DEA number.
- Previous experience in nursing or other medical fields.
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast‑paced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience with emphasis on teamwork in a healthcare setting.
- Master’s degree required.
