Job Summary

Under the supervision of the Director of Population Health, the Chronic Care Navigator will support chronic care management efforts by providing telephonic and in‑person support to patients with chronic conditions. Responsibilities include identifying barriers to accessing health care, providing social support and health education, and serving as a liaison between individuals, communities, and health & social services. The role requires working in both clinical and community‑based settings, including client homes.

  • Fluent written and verbal skills in English and Spanish (preferred).
Job Requirements
  • High school diploma plus specialized, short‑term training in clinical or medical skills.
  • Current, nationally recognized Medical Assistant certification (required).
  • Certified Community Health Worker certification preferred.
  • 2+ years of experience in a physician’s office, clinic, or hospital setting.
  • Proficient computer skills for efficient use of electronic medical records.
