Youth & Family Navigator

Full-time, non-exempt, hourly position at Families & Youth Innovations Plus.

We are hiring a Youth & Family Navigator to greet and navigate the needs of individuals seeking services at FYI+. The role harmonizes multiple systems of response through referral systems, service coordination, screening, and triage. It also involves outreach, community relationship development, and ensuring customer service through a trauma-informed lens.

Key Responsibilities
  • Provide confidential administrative support related to sensitive issues.
  • Triage customers’ needs and guide them to the appropriate pathway of services.
  • Assist customers in completing onboarding paperwork and filling out applications.
  • Coordinate warm hand-offs between providers for each pathway to services.
  • Serve as a community referral link for customers to internal and/or external services and service providers.
  • Schedule new patients for intake assessments with therapists daily.
  • Monitor scheduled intakes daily, reschedule as needed.
  • Manage agency data tracking systems for all referrals through the Resource and Referral database.
  • Manage NM Star database and record all required data relevant to Treat First.
  • Verify private insurance and Medicaid eligibility via NM Medicaid portal.
  • Utilize EMR clinical database to activate new clients and manage referrals for the agency.

Qualifications and Experience
  • Bachelor’s degree in a human service field from an accredited university and one year relevant experience working with the target population; or associate’s degree and two years of experience.
  • Microsoft Office Suite (Intermediate).
  • Excellent written and verbal communication skills.
  • Provide services in the most culturally sensitive, appropriate and ethical manner.
  • Exhibit skills in comprehending, interpreting, and completing delegated tasks efficiently.
