FieldThis field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as needed, with occasional office attendance for meetings or training.
Job DescriptionThe Community Wimaker is responsible for serving as a liaison/connector between the patient, the care team and the community. The role focuses on bridging conversations with patients, removing barriers that prevent access to health and social services, and conducting telephonic and/or face‑to‑face outreach for appointment scheduling, needs assessment, and care gap closure.
Responsibilities- Meet patients at the clinic or facility to identify social determinants impacting health and well‑being.
- Collaborate with social workers and/or care managers to develop action plans.
- Develop trusting relationships with patients by providing support and advocacy.
- Assist patients in accessing health‑related services and community resources, including accompaniment to specialty appointments and aid with enrollment forms.
- Facilitate communication betweenergedall parties (patients, families, colleagues, and community organizations).
- Document interactions with and on behalf of patients in the medical record.
- Attend meetings as required.
- Travel to worksites and other locations as necessary.
- High school diploma or equivalent; experience using an electronic medical record; one year of experience in a health, community‑based, or social work environment, or a comparable combination of education and experience.
- Bilingual in English and Spanish (required).
- Certified Community Health Worker (preferred).
- Experience navigating challenges related to homelessness and employment (preferred).
- Reliable personal transportation (preferred).
