The Division of Child and Family Services is recruiting for a Psychiatric Caseworker 1 to fill in for a Psychiatric Caseworker 2 in the Child & Adolescent Mobile Crisis Response Team. Psychiatric Caseworkers work in teams with a mental health therapist and complete targeted case management assessments, recommendations, and care coordination services at the location of family’s needs. The Mobile Crisis Response Team receives, triages calls from families and community partners, and responds in teams to families with children experiencing a behavioral health crisis.
Responses can take place in emergency departments, schools, homes, and other community settings. Incumbents are responsible for completing a targeted case management assessment, providing support, immediate assistance, and creating plans jointly with the mental health therapist, youth, and family to ensure the safety of the youth and to maintain them in their homes, schools, and communities when able. Incumbents provide case‑management services to children and adolescents in crisis in an outpatient clinic or community setting.
Teams can remain in place for up to 45 days to provide clinical and case‑management services to families. This position will be the case‑management lead on the Mobile Crisis Response Team, while mental health counselors take the clinical lead.
- Perform casework duties and monitor progress of clients in treatment.
- Assess and reassess clients’ level of functioning through interviews with clients, family members, and other community contacts.
- Evaluate clients’ needs and resources to determine the level of assistance required and refer clients to appropriate services.
- Coordinate services, make referrals to other service providers and community resources, and discuss mental and emotional conditions, illnesses and disorders, family issues, and developmental delays.
- Interview clients for psychosocial rehabilitation program placement, determine training needs, and assign to appropriate groups/classes.
- Prepare written evaluations of the client and service/care coordination plans for each service.
- Enter progress notes and advise clients of their service‑related rights and responsibilities.
- Develop treatment plans and participate in multidisciplinary team meetings to determine client goals and develop modified strategies and plans.
- Integrate and analyze information from all available sources and systems; coordinate and integrate services among china team members and client support systems.
- Monitor treatment plans and evaluate effectiveness, documenting quality of care and ensuring information is available to the multidisciplinary team.
- Provide ongoing individual and family managementтим education regarding daily‑living issues, coping skills, and employment; provide information to client’s family members or other caretakers regarding clients’ rights and actions necessary to obtain optimum benefits.
- Assist clients, families, and care providers in understanding the nature and treatment of clients’ illness or condition.
- Develop and implement discharge plans; prepare required pàort documentation; schedule appointments such as at the medication clinic.
- Obtain information from clinical professionals concerning clients; make referrals to appropriate clinicians; refer to community resources and make housing referrals/Object.
- Write client housing contracts according to established procedures; monitor supported housing arrangements and delivery of services to ensure compliance with the terms and conditions of the contract.
- Monitor follow‑up services specified in the treatment plan, including attendance at medication clinics, day treatment programs, outpatient appointments, vocational/rehabilitation services and housing arrangements to reduce readmission.
