Overview
LVN/LPN-H (Healthcare)

- Administrative Functions:
Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility. Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility. Periodically review the department's policies, procedure manuals, job descriptions, etc., and make recommendations for revisions. - Program Planning:
Meet with your assigned nursing staff and support personnel to plan shifts, services, programs, and activities. Ensure the Nursing Service Procedures Manual is current and reflects day to day nursing procedures; ensure compliance with its procedures. Make written and oral reports/recommendations concerning shift activities. Cooperate with other resident services to maintain the resident’s total regimen of care. - Discharge Planning and QA:
Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing service department. Participate in facility surveys as requested. Periodically review and update the resident’s discharge plan and assist in planning the nursing services portion of the discharge plan as necessary. - Policy Interpretation and Compliance:
Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required. Admit, transfer, and discharge residents as required. Complete accident/incident reports and maintain related documentation. Maintain the Daily Census Report and submit to the Business Office as required. Complete administrative forms, reports, evaluations, studies, charting, etc., as necessary.
- Security and Confidentiality:
Agree not to disclose assigned user /password and to report violations. Protect resident health information and report unauthorized access attempts. Ensure proper log-off or password protection of unattended computer workstations.
- Complete and file required recordkeeping forms upon admission, transfer, and discharge. Encourage physicians to review and sign orders and progress notes. Receive telephone orders and transcribe to charts. Document nursing notes descriptively and reflect care provided and resident responses. Fill out and file accident/incident reports and transfer forms as required. Record diet orders and communicate with Food Services. Resolve discrepancies in orders or charting with the Nurse Supervisor.
Ensure documentation of unauthorized discharges when applicable. Perform routine charting per policies and sign all entries.
- Prepare and administer medications as ordered. Verify resident identity before administration. Maintain floor stock, order medications, and ensure narcotic records are accurate for the shift. Notify the Nurse Supervisor of discrepancies. Review medication cards for accuracy and stop orders. Dispose of drugs per procedures and follow stop orders.
- Participate in evaluations, determine staffing needs, and make recommendations. Notify the Nurse Supervisor of absences. Develop work assignments and provide leadership. Perform daily rounds to ensure standards are met. Ensure CNAs are enrolled in approved programs and supervised. Meet regularly with nursing personnel to address issues, ensure policy compliance, and foster teamwork. Maintain positive interdepartmental relationships and promote a warm, calm unit environment.
Review complaints and grievances; report as required. Ensure disciplinary actions are fair and non-discriminatory. Report occupational exposures and fraud as required. Ensure security of departmental computers and data integrity.
