• You will ensure high quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes
• As an active member of the rehab team, you will provide, coordinate, and direct care specific to the needs of each Rehab patient
• You will collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions, and coordinate efforts within the rehabilitation team
• You will provide thorough verbal and/or printed information to help patients/families/caregivers make informed decisions about post-acute care options, while addressing goals of care and treatment preferences
• You will facilitate discharge and arrangement of follow-up services, implementation of discharge/transition recommendations, and identify and utilize appropriate community resources to meet patient discharge and continuity of care needs

• Associate Degree in Nursing or RN Diploma required
• Bachelor's Degree in Nursing preferred
• 1+ years in acute care or inpatient rehab required
• Experience in case management preferred
• Current license as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation required
• Certified Rehab Registered Nurse (CRRN) preferred
• Certification in Case Management preferred
• BLS required
