What You’ll Do- Provide Direct Patient Care:
Administer treatments and medications as prescribed, develop individualized care plans, and offer compassionate guidance to patients and families. - Make a Real Difference:
Complete comprehensive assessments, evaluate patient conditions, and ensure care plans are aligned with evolving needs to ensure comfort and dignity at every stage. - Collaborate with an Expert Team:
Work closely with physicians, social workers, and other healthcare professionals to ensure optimal patient outcomes and provide holistic, patient‑centered care. - Coordinate Care:
Lead efforts as a case manager for your assigned patients, ensuring seamless, high‑quality care across all aspects of the patient’s journey. - Teach & Empower Families:
Provide education on self‑care techniques, symptom management, and how to ensure patients are as comfortable and independent as possible.
- Competitive Pay:
Earn $38-57 per hour based on your experience. - Feel the Impact:

- Registered Nurse (RN) with a valid license in your state.
- Experience in Hospice or Home Health Care:
Minimum of 1 year of professional nursing experience; hospice experience is required. - Compassionate & Empathetic: A heart for helping others, especially those facing end‑of‑life challenges.
- Strong Communication
Skills:
Ability to coordinate care with physicians, patients, and families, and clearly document clinical notes. - BSN Preferred:
Experience or education in the aged, chronically ill, restorative, or psychiatric nursing preferred.
- Graduate from an accredited nursing school (BSN preferred).
- Valid RN license in the practice state.
- Active BLS certification.
- A licensed driver with an insured automobile.
