Job Description

As a Home Health Registered nurse, you’ll provide compassionate care in the comfort of patients’ homes. You’ll be responsible for assessing, planning, and delivering care, while ensuring patients receive the best possible treatment in a home health setting. You’ll work independently, using your nursing skills to follow best practices and stay in line with home health guidelines. This role includes utilizing technology, providing therapeutic interventions, and collaborating with other healthcare team members to coordinate care.

Position Details

Key Responsibilities
  • Provide nursing assessments, interventions, and evaluations for adult and pediatric patients in their home setting.
  • Develop personalized care plans to meet the physical, emotional, and psychological needs of patients.
  • Use the nursing process to ensure care is delivered effectively, efficiently, and in compliance with policies and regulations.
  • Provide education and support to patients and their families to help them understand and manage their health conditions.
  • Collaborate with other members of the healthcare team to coordinate and deliver care.
  • Document all care provided in compliance with home health guidelines.
  • Ensure a safe and healing environment for patients, helping them feel supported and cared for in their home.
Minimum Qualifications
  • Current RN License in the state of practice.
  • Basic Life Support (BLS) Certification for healthcare providers.
  • A valid driver’s license, insured, reliable transportation, and an acceptable driving record.
  • Ability to communicate effectively, both verbally and in writing.
  • RNs with less than 12 months of working experience as an RN must obtain their BSN within 4 years of their start date.
Preferred Qualifications
  • Bachelor’s Degree in Nursing (BSN)
  • Two years of experience as an RN.
  • Experience in Acute Care, Home Health, or Hospice settings is a plus.
Physical Demands
  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.
