• $41.50–$52.50 per hour

• Help patients maintain, restore, and improve movement, activity, and function.
• Provide interventions, conduct re‑examinations (every 30 days minimum), modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.

• Current license to practice physical therapy in the state of assignment.
• Graduate from an accredited physical therapy program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year of related physical therapy experience for PTs entering home‑health care; if less than one year, approval from regional clinical leaders is required along with appropriate mentoring or residency.
• Current CPR certification.
